Community Connections Graduates
Community Connections assists students with learning needs as they transition from high school to adult living and post-secondary education after high school graduation. Graduates learned important life skills, explored careers and became involved with the community.

C.A.M.P.U.S. at CSU Program
C.A.M.P.U.S is a partnership between PSD Transition Pathways Program, CSU Transition, Employment, and Technology Lab, and the CSU Center for Community Partnerships. This collaboration provides opportunities for students with disabilities, ages 18-21, to experience transition-based activities to continue developing their independence, self-advocacy, employment, and social integration skills on the university campus.
A celebration honoring graduates was held May 15.
Pratiti Santra
Attended Fort Collins High School and community connections before finishing with C.A.M.P.U.S. pathway. Pratiti will continue to engage with her community through volunteer, recreation, and other engagement activities.
Rosemary Chaparro
Attended Fort Collins High School and Cooper Home before completing the C.A.M.P.U.S. pathway. Rosemary will continue working at her paid employment Pups on the 51风流 so that she can save up to live with a roommate.
Desirae Chmielowiec
Attended Fossil Ridge High School and Cooper Home before completing C.A.M.P.U.S. Desirae will continue her paid position at the MetroLux Theater and one day hopes to own her own movie theater.
Jackson Statkus
Attended Fossil Ridge High School and Project Search before completing C.A.M.P.U.S. Jackson will have a paid summer position with Genesis Health Club and then will continue to look for employment opportunities and possibly enroll in an early childhood program.
Cooper Home Graduates
Cooper Home assists students in moving toward natural supports within the community and the worksite while fostering independence. Services include job development and coaching, training in work-related skills, independent living preparation, daily living and social skills training and more.
A celebration honoring graduates was held May 21.
Scout Grieves
Scout plans to continue working at Fossil Ridge High School after graduation and will also help his family with anything and everything they might need. Scout has shown incredible work ethic and a wonderful attitude every day he has attended Cooper Home.
Meghan Kenemuth
Meghan will pursue further education in the culinary arts, as well as explore opportunities in the creative arts field. Meghan has grown so much during her time at Cooper Home. We are so excited to see where her journey takes her! She is truly a creative powerhouse.
Project SEARCH Graduates
Project Search is a business-led collaboration that enables young adults with disabilities to maintain employment through career exploration and training.
A graduation celebration recognizing students’ growth and the program's16-year partnership with Columbine Health Systems was held May 22.
Rielle Artis
Rielle is eagerly searching for a job after successfully completing two internships. She has incredible people skills, and she would like to work in retail.
Alfie Chamberlain
After successfully completing three different internships, Alfie will continue in the school district by attending the CAMPUS Transition Pathway. He is currently enjoying his work as a seasonal employee at Bath Garden Center.
Kasia Charkowski
After successfully completing two different internships, Kasia will continue in the school district by attending the Cooper Home Transition Pathway. She is also pursuing her writing.
Jindi Gueswel
Jindi is looking for a job in a bakery after successfully completing two internships. She has maintained a part-time job at Cups Community Coffee for the past two years.
Logan Hitti-Frazen
Logan will soon begin his job hunt after successfully completing two internships at Columbine Health Services. He would like to own his own pizza restaurant one day.
Greta Jones
After successfully completing two internships, Greta is in the process of finding the right job. She is interested in a people-oriented position to show off her numerous strengths.
