Notifications for Vaccination Information

  • The 2024-25 immunization letter for parents was emailed to 51风流 families on Feb. 13, 2024. The letter, available in all languages, is also available on the :

  • Exclusion day was Nov. 1, 2023. Any student without proof of required immunizations or a valid exemption form on file with the health office is excluded from attending school until the requirements are met. A Notice of Exclusion letter was emailed Oct. 9, 2023, to let parents know to look for the notices in ParentVUE.
  • PSD emailed families of students with missing vaccination information the Colorado Notice of Requirement on Sept. 18, 2023.

Requirements

All students attending Colorado schools and licensed childcare centers are required to be vaccinated against certain diseases, unless an exemption is filed (see below), according to . The minimum number and spacing of doses is set by the Advisory Committee on Immunization Practices (ACIP) Child and Immunization Schedules.

Meeting the initial vaccine requirements does not excuse a student from meeting additional requirements. In addition to the vaccines required for school entry, the ACIP recommends several vaccines that provide protection against other diseases. These include:

  • Meningococcal
  • Hepatitis A
  • Rotavirus
  • Human Papillomavirus
  • Influenza
  • COVID-19

An up-to-date immunization record, in-process plan, or exemption must be submitted to your school’s health office within 14 days of notification of requirement. In the event of continued immunization non-compliance, students will be excluded from the school setting.

Required Vaccines

For Preschool

  • Hepatitis B
  • Diphtheria, tetanus, pertussis (DTap)
  • Haemophilus influenzae type b (Hib)
  • Inactivated poliovirus (IPV)
  • Pneumococcal conjugate (PCV13 or PCV15)
  • Measles, mumps, rubella (MMR)
  • Varicella (chicken pox)

For K-12 Grades

  • Hepatitis B
  • Diphtheria, tetanus, pertussis (DTap)*
  • Inactivated poliovirus (IPV)*
  • Measles, mumps, rubella (MMR)*
  • Varicella (chicken pox)*
  • Tetanus, diphtheria, pertussis (Tdap)**

*Students 4 through 6 years of age are required to have their final doses of DTaP,  IPV, MMR and Varicella prior to kindergarten entry.

**Students are required to have Tdap prior to entry into 6th grade. It is acceptable to give as early as age 10. Only one dose of Tdap is required for 6th through 12th grades after the primary DTaP series is complete. 

Immunization Exemptions

Nonmedical Exemptions

Please note that nonmedical exemptions expire each year on June 30.

You may submit a nonmedical exemption (paper copy or by email) annually to your school's health office in one of two ways, which include:

1) Submit the Certificate of Nonmedical Exemption received upon the completion of to your school’s health office.

  • We do not recommend using a mobile device or tablet to complete the course and form.
  • The Online Immunization Education Module is available 24 hours a day and seven days a week and takes approximately 20 minutes to complete. 
  • After completing the module, you will complete an electronic form to obtain a Certificate of Nonmedical Exemption. You can complete this form for multiple children, by selecting the 'back' button and resubmitting the exemption each time it appears.
  • Parents of students in preschool or child care must submit nonmedical exemptions at 2, 4, 6, 12 and 18 months of age. These exemptions expire when the next vaccines are due or when the child enrolls in kindergarten.

2) Submit the Certificate of Nonmedical Exemption with a signature from an immunizing provider in Colorado who is a medical doctor, doctor of osteopathic medicine, advanced practice nurse, delegated physician’s assistant, registered nurse, or pharmacist who has entered this information into the Colorado Immunization Information System (CIIS).

Medical Exemptions

Students with a completed signed by an advanced practice nurse, a delegated physician assistant, or physician licensed to practice medicine or osteopathic medicine in any state or territory of the United States need to only submit their certificate once unless the student’s medical information or school district changes.

 

Immunization/Exemption Rates by School

Schools and childcare centers are required to send information about a school’s immunization rates each year. You can find immunization rates by school for Diphtheria, tetanus, and pertussis (DTap, Tdap); Hepatitis B; Polio; and Varicella on the (select the orange K-12 button and select a school from the drop-down menu).
